A medical transcriptionist was listening to a doctor dictate a report on a patient being evaluated for an industrial injury to his right knee. The doctor very clearly dictated that he had x-rayed the right knee and aspirated the right knee.
Later in his dictation, the physician clearly stated that he had administered a cortisone injection into the patient's left knee.
"I flagged the report for review,"recalls the medical transcriptionist," but later, the office manager indicated that he would not be sending this back to the doctor for review and revision because he just didn't want to deal with it. The chart indicated the problem was with the left knee and I was instructed to revise the report accordingly."
