Mountains of Paperwork

Third-party insurers are now calling the shots, forcing doctors to call an anonymous administrative assistant for approval of a patient's treatment plan. The mess of paperwork is made worse by a web of conflicting instructions from various insurance carriers.

It would be nice to think that the HCFA 1500 was the only form that had to be completed. But the HCFA-1500 comes in several different formats, with little standardization from one insurance carrier to the next.
